MASH Major Incident Reporting Policy

It is the policy of the Minnesota Association of Sober Homes (MASH) that all certified organizations promptly report any major incident occurring within a certified recovery residence or involving affiliated staff, volunteers, or representatives.

A major incident is defined as any emergency or critical situation that significantly impacts resident safety, organizational operations, or public trust, and requires a response beyond routine incident management. Major incidents also include — but are not limited to — violations of the MASH/NARR Code of Ethics by any team member (including staff, owners, volunteers, or peer leaders), especially when such actions directly or indirectly affect residents or participants.

If a major incident occurs, it is the responsibility of the certified organization to complete and submit the MASH Incident Report Form, along with a written summary of the event, within five (5) business days of the incident.

Incident reports must be submitted to the MASH office for review. When necessary, MASH may refer the report to its Board of Directors or a designated review committee for further investigation. MASH maintains records of all submitted major incident reports and reserves the right to request additional documentation or corrective actions as part of ongoing certification oversight.
